What Is Ostomy Irrigation?
Ostomy irrigation is a technique used by some people with colostomies to regulate bowel output by introducing water into the colon through the stoma at a set time each day. When performed consistently, irrigation can allow the colon to empty on a predictable schedule, often eliminating spontaneous output between irrigations. This allows many irrigators to wear only a small stoma cap or security pouch rather than a full drainage pouch during the day, which can significantly improve comfort, discreteness, and quality of life. Irrigation is not suitable for everyone — it is typically only appropriate for people with a sigmoid or descending colostomy, and should only be started under the guidance of an ostomy nurse or healthcare provider.
Irrigation System Components
A complete irrigation setup consists of several components: an irrigation bag or reservoir that holds the water, a flow control mechanism to regulate water delivery, a cone tip that is inserted into the stoma (replacing the older catheter method, which is no longer recommended), an irrigation sleeve that fits over the stoma and directs output into the toilet, and a belt to hold the sleeve in place. Most systems are sold as complete starter kits that include all required components, with individual replacement parts available separately for ongoing use. The cone tip is the most frequently replaced component and is available separately across all major brands.
Hollister Irrigation Products
Hollister offers the closed-end and open-end irrigation sleeves, irrigation bags, cone tips, and complete irrigation sets under their ostomy product line. Hollister irrigation sleeves are available in multiple lengths to accommodate different body heights and toilet configurations. Replacement cone tips and bags are sold separately for users who have already established their irrigation routine with Hollister equipment.
Coloplast Irrigation Products
Coloplast's irrigation line includes the Peristeen and Alterna irrigation systems. The Alterna irrigation set includes the reservoir bag, tubing, cone, and sleeve as a complete system. Coloplast also offers individual replacement sleeves, cones, and bags. The cone tip design is intended to create a secure, comfortable fit at the stoma opening without requiring insertion into the bowel.
Finding Your Product
If you are setting up an irrigation routine for the first time, start with a complete irrigation kit rather than purchasing components individually. Your ostomy nurse will guide you on technique, water volume, and timing. If you are an established irrigator reordering supplies, use the brand filter to find your specific system's replacement parts — irrigation components are not interchangeable between brands. If you are unsure whether irrigation is appropriate for your stoma type and overall health, consult your ostomy nurse or surgeon before purchasing.
Ostomy Irrigation FAQ
Who is ostomy irrigation suitable for?
Ostomy irrigation is generally suitable for adults with a sigmoid or descending colostomy who have formed, predictable output and are in good overall health. It is not recommended for people with ileostomies or urostomies, those with Crohn's disease, diverticular disease, radiation-damaged bowel, or other conditions affecting the colon. It is also not typically recommended for children or for people who have had their colostomy for less than six to eight weeks. Suitability is assessed individually — your ostomy nurse will evaluate your stoma type, bowel function, manual dexterity, and overall health before recommending irrigation as an option.
How long does ostomy irrigation take?
The full irrigation process typically takes 45 to 60 minutes, including water instillation and the waiting period for output to complete. Water instillation itself usually takes about 10 minutes; the subsequent drainage period takes another 20 to 30 minutes. Most experienced irrigators complete the process in under an hour. Irrigation is typically performed once every 24 to 48 hours, at the same time each day, to establish a consistent bowel routine. It may take several weeks of consistent practice before a reliable pattern is established. Your ostomy nurse will guide you on timing and technique when you first start.
Can I use irrigation equipment from one brand with another brand's components?
No — irrigation components are designed to work within their own brand's system and are not interchangeable between manufacturers. The cone tip, tubing connectors, and sleeve attachment mechanisms differ between Hollister and Coloplast systems. If you are reordering replacement parts, always match the brand and product line of your existing system. If you are switching brands entirely, purchase a complete new kit rather than mixing components, and have your ostomy nurse review your technique with the new system before using it independently.
What are the advantages of irrigation compared to wearing a standard pouching system?
For suitable candidates who irrigate consistently, the main advantages are predictability and discreteness. Successful irrigators can often go without a full drainage pouch between irrigations, wearing only a small stoma cap or security pouch — which is less bulky, less visible under clothing, and associated with improved body image and quality of life in many users. Irrigation also eliminates concerns about unexpected output during daily activities. The trade-off is the time commitment of the irrigation procedure itself and the need for consistent technique. Irrigation does not work for everyone, and some users find that a standard pouching system better fits their lifestyle even when irrigation is technically suitable for them.
How often should irrigation equipment be replaced?
Irrigation sleeves are typically single-use or short-term use products, depending on the specific product and manufacturer guidance — check the instructions included with your sleeve for the recommended usage period. The cone tip is a reusable component that should be cleaned after each use and replaced periodically as recommended by the manufacturer or when it shows signs of wear or deterioration. The irrigation bag and tubing are typically reusable for an extended period with proper cleaning and storage. Always follow the specific maintenance and replacement guidance in your product's instructions for use, and replace any component that appears damaged, discoloured, or difficult to clean.
