What Is a 2-Piece Urostomy Pouch?
A 2-piece urostomy pouching system consists of two separate components: a skin barrier (wafer) that adheres to the skin around the stoma, and a urostomy pouch that attaches to the barrier via a coupling mechanism — either a plastic flange that snaps together or an adhesive seal. Because the two parts connect and disconnect independently, you can change the pouch without removing the skin barrier from your body. Urostomy pouches are specifically designed to collect urine from a urostomy (ileal conduit or similar diversion), and feature a tap or valve at the bottom for draining rather than a tail closure.

How a 2-Piece System Differs From a 1-Piece
In a 1-piece system, the skin barrier and pouch are permanently bonded and changed together as a single unit. In a 2-piece system, the barrier stays on the skin for several days while pouches can be swapped individually — typically daily or as needed. This separation offers several practical advantages: the skin barrier experiences less disruption and can often be worn longer, pouch changes are quicker and require less skin manipulation, and some users find it easier to attach the pouch while sitting or lying down. The tradeoff is a slightly bulkier profile due to the coupling mechanism between the two components.

Flange Coupling vs. Adhesive Coupling
Two-piece urostomy systems connect via one of two methods. Flange coupling uses a rigid or semi-rigid plastic ring on both the barrier and the pouch that snap together with an audible click, providing a secure and easily verifiable connection. Adhesive coupling (sometimes called soft coupling or flex seal) uses a flexible adhesive ring that presses together without a hard ring, creating a softer, lower-profile connection that moves more naturally with the body. Flange systems are generally easier to confirm as correctly attached; adhesive coupling systems tend to be more discreet and flexible. Your stoma nurse can help you determine which coupling style suits your anatomy and lifestyle.

Urostomy Pouch Features to Look For
Urostomy pouches include a drain tap or valve at the bottom for emptying without removing the pouch — most users empty every 2 to 4 hours during the day and connect to a larger overnight drainage bag at night. Look for pouches with an anti-reflux valve, which prevents urine from flowing back toward the stoma when the pouch fills or when you change position. Most urostomy pouches also include a soft backing fabric on the skin-facing side for comfort, and are available in transparent or opaque film to suit personal preference.

Finding Your Product
Use the brand filter to narrow by manufacturer. Filter by price or search by product name or SKU to find a specific item. Flange size is critical in a 2-piece system — the flange size on your pouch must match the flange size on your skin barrier exactly. Check your existing barrier packaging for the flange size before ordering pouches separately. If you are setting up a new system, your ostomy nurse will measure your stoma and recommend the appropriate flange size.

2-Piece Urostomy Pouch FAQ

What is the difference between a urostomy pouch and a drainable ostomy pouch?
A urostomy pouch is specifically designed to collect urine from a urinary diversion such as an ileal conduit. It features a drain tap or valve at the bottom for controlled emptying and typically includes an anti-reflux valve to prevent urine from flowing back toward the stoma. A drainable ostomy pouch is designed to collect stool output from a colostomy or ileostomy and uses a tail closure — a clamp, clip, or fold — rather than a tap. The two are not interchangeable; urostomy pouches should only be used with urostomies, and drainable stool pouches should not be used for urinary diversions.
How often should a 2-piece urostomy pouch be changed?
In a 2-piece system, the skin barrier and the pouch are changed on different schedules. The skin barrier is typically worn for 3 to 5 days before being replaced, depending on the individual's skin condition, output, and the specific product used. The urostomy pouch itself can be changed daily or as needed without disturbing the barrier. Most urostomy pouches are designed for single-day use. Always follow the instructions for use included with your specific product, and replace the barrier sooner if you notice lifting edges, leakage, or skin irritation.
What is an anti-reflux valve and why does it matter?
An anti-reflux valve is a one-way valve inside the urostomy pouch that allows urine to flow down into the pouch but prevents it from flowing back up toward the stoma when the pouch is full or when you change position — such as lying down or bending over. Without this valve, urine sitting in a full pouch could come into contact with the stoma and the peristomal skin, increasing the risk of skin irritation and urinary tract complications. Most urostomy pouches include an anti-reflux valve as a standard feature; check the individual product listing to confirm.
Do I need to match the flange size when buying a 2-piece urostomy pouch?
Yes — flange size matching is essential in a 2-piece system. The coupling size on the pouch must match the coupling size on your skin barrier exactly for the two components to connect correctly and create a secure seal. Common flange sizes include 32mm, 38mm, 45mm, and 57mm, though sizes vary by manufacturer. Check the packaging of your existing skin barrier for the flange size before purchasing pouches separately. Flange sizes are not always interchangeable between brands, so it is generally recommended to stay within the same manufacturer's product line when pairing barriers and pouches.
Can I connect a 2-piece urostomy pouch to a night drainage bag?
Yes — most urostomy pouches are designed to connect to a larger overnight drainage bag via the drain tap at the bottom of the pouch. This allows urine to drain continuously into a bedside bag while you sleep, eliminating the need to get up during the night to empty the pouch. Ensure the connector on the overnight bag is compatible with the drain tap on your urostomy pouch before purchasing. Night drainage bags are sold separately and are listed in the Ostomy Accessories section of our store.
